A friend has obtained an 8146A OTDR --- it complains about a "safety failure" warning upon powerup. The agilent web site has literally nothing on this box at all. Any help would be appreciated.

The 8146A is rather old and it may be hard to find a manual (if your friend didn't get one with the unit). Possibly there is some similarity to the 8147A with this manual:
The "laser safety" warning at boot-up sounds like the laser is not able to achieve the design power level without exceeding the "safe" current level.
This may mean the laser module is no longer working, if this always occurs.
